Error uploading SkinStudio Widget

Above is the what was showing in the addressbar after the upload....it still went to the page saying.....wait five days...yada yada, but when I went to MySkins it didn't show up as 'Waiting Approval'
Please look at the info I entered in the upload page to see where I goofed up so that I can fix and try to upload again....Thanks All

* Absolutely no text showing
* No UserIcon...so that you don't have to ask folks to turn it off in your description (make sure your using the latest SkinStudio for this one to work)
Just Unzip in your SkinStudio Widgets folder
This widget is for WinXP only and WB 4.2

Also tried changing the name of the zip and jpg file and removing the *'s from the comments....nothing, nada, nil

I even tried from my wife's computer

Now that Easter is over and it's Monday, I'm hoping that someone can help me see the light and figure out why I can't get this thing to upload
