Additional menu icons not showing proper state.

I'm trying out the additional menu icons for the first time. I need them to closely emulate the default windows 8 menus as closely as possible and have run into an issue. The "states" aren't showing correctly (or as directed in the help section of sks). Only the radio box shows. Not the check boxes.
The issue is illustrated here in this screeny:

This is what happened last time I submitted a ticket to support (quoted from another post)
I submitted a ticket regarding this issue (linking to this post also to save time as I have outlined the issue here with pictures) and followed the instructions provided by support (download a thingy to gather info about my computer) then heard nothing for 5 days.
I went back to work (I work out at sea and am without internet for longish periods of time) and meanwhile my ticket was closed after not replying within 72 hrs.
Too bad that I'm expected to wait for 5 days before receiving a reply, but support closes the case after 3 days of not hearing anything.
Doubtful that I'll be resubmitting a ticket on this issue given that I cannot guarantee that I can reply before my ticket is closed again.

I did send a PM to Neil but I cannot ever remember him replying to any PM's I've sent him in the past.......

I don't know what happened to Neil, it's like he dropped off the face of the Earth Tim. Sorry to hear about your support ticket issue in the past, but I don't know what else you can do at this point.
