Fault Module Name: ad2mpegin.dll
search the .dll file, it's an MPEG codex (decoder) could be associated with a number of media apps.
some of which are Adobe, like Premier Elements

Adobe installed? Yes, CS4.
mastersuite? or at least a bundle with Premier Elements?
are you running a 64 bit OS?
not all Adobe product are "officially" 64bit compatible. "officially" as in Adobe claims they run on 64bit but some apps still have bugs at times, Adobe has not yet worked out all the bugs that occur on 64bit OS, due to this they have not yet marked those apps as 64bit compatible and will not offer support on those apps.
if running a 64bit OS that may be the problem.
I would suggest uninstalling CS4, running a registry cleaner, and reinstalling to see if that helps.

Today I found the dll and deleted it, and still had a crash, rebooted, none since...so far.
