Icon Issue

Having a weird problem of late. If you look at the screenshot below, most of my application icons have the default icon from any/all IP packages I apply associated with them. I've done the rebuild icon cache, etc., all with no luck. I've gone back to the default vista icons and this doesn't fix the issue. One final thing I still have to do is uninstall/reinstall IP, but thought I'd ask in case anyone else has had this problem in the meantime.
