Titlebar and window border disappears on Vista

I don't have a screenshot atm, but it's not that hard to image. The titlebar (has the name of the window and the minimize, maximize, and x buttons) along with the surrounding border seems to randomly disappear. I don't want to jump the gun but from what i've noticed it seems like it only does it when a program is opened via something other than me. Example: I download a .torrent file and tell firefox to open with utorrent. When utorrent opens I get what is inside of the utorrent window, fully functional, but the border and titlebar are gone. They seem to still function. If I guestimate where the buttons are I can successfully x, min, and max the window. Even resize it. But the fact that it's not there bugs me. Anyone else have this problem? If it happens again and I remember I'll take a screenshot.
