Applied Set Changes Back To Default

Out of the blue today, when I decided to change my icon set, Icon Packager is acting up.
I apply a set and when I open a window I see it for a few seconds then, one by one, change back to the default icon.
I've tried Rebuild Icon and Repair Icon, both change them back to the chosen set only to have them change back a sec or two later. I have checked that I have the latest version, I've tried several different icon sets, I'm running Vista and things had been working ok until today. Any help greatly appreciated. Thanks.

It's the latest available - 3.20 enhanced, paid for, has been for years.
