crashes in cache.dll

It has done this with the last few versions.
Any clues?

I use Litestep and if I change icons via IconPackager [or upgrade], Windows Explorer doesn't start. The process does, but the application does not.
Sometimes uninstalling/re-installing IP cures this, other times not.
Tried going back to Explorer shell and doing the same - again hit and miss whether it works or not.
Damned frustrating.
The thing is when it works, all the icons are correct and all is as it should be, but an upgrade or icon change really screws it up.
Any suggestions would be gratefully received...

Which version of IconPackager and Windows (including service packs) are you running?
