If you still have problems, you may want to save your package themes to a temporary folder > Restore default icons > uninstall IconPackager > reboot > delete the IconPackager directory > reintstall IconPackager.
If all goes well, copy or move saved package themes back to IconPackager directory.

Normally, you would start Icon Packager, go to "Tools > select "Restore all Icons" > click "Yes" button > click "OK" in Icon Packager dialog.
After a few seconds, the dialog will close, and all the icons should be returned to their defaults.
Now you can start Icon Packager again to select and apply a new package.
Note - there are also two functions under the Tools menu "Rebuild Icon Cache" and "Repair Icon Images" which can be used to help fix issues.
