Icons not changing?

You can try running the program and using the "Tools>Repair
Icon Images", then clicking "OK".
If that fails, you could try the following:
Using IconPackager tools menu, perform the following: "Restore all icons", click "OK" > "Restore all cursors", click "OK" > "Rebuild Icon Cache", click "OK" (this is the official support method to restore system to defaults from Icon Packager).
Restore the Windows default theme using Display Properties dialog (if using XP, just right-click desktop and select "Properties", then on the "Themes" tab - select "Windows XP" from the theme drop down list, then click "Apply").
Reboot the computer, and run IconPackager to apply the package you want.
If the problem persists, I would suspect either a limitation in the free verion (which you mentioned) or a corrupted install of the program. I did not see anything in the description for IconPackager that would indicate a limitation for this feature in the free version, but it may be.
If you are feeling adventurous, you could always uninstall, reboot, delete the IconPackager folder in "C:\Program Files\Stardock" folder, download a fresh installer file, reinstall IconPackager - then see how it goes.
Again, this may be pointless, but if you have nothing else to do....

I had to boot into safe mode to get rid of CursorXP (i swear, that program hates me), and all the files were changed and everything. So that makes me think it's another program. Could it be LiteStep?

Yep..... one of the reason I didn't use LiteStep for long was the incompatibility with IconPackager.
