"Blank" Icons in Start Menu (not refreshing either)

After installing Icon Packager I have two issues:

1) Most of my icons are now "blank", just this white paper looking thing with the top corner folded.

2) Clicking on Start -> All Programs -> ... sometimes (not always) when I click on a folder it doesn't "open" to show the contents. Instead the folder turns into a little magnifying glass thingy. If I close the Start Menu and click on Start -> All Paograms -> then the folder is now "open".

This is on a Windows Vista system that has been running just fine for well over a year. I install Icon Packager today, and now I am seeing this so my assumption is that this is the issue. I have disabled / uninstalled everything back to 'vanilla' Vista but my icons are still the same (blank).

System Info (components that my be helpful):
Windows Vista Ultimate 64-bit
Nvidia GTX 280 SLI
