Icon Cache gone, missing, disappeared

My issue is that after installing Object Desktop on my Vista x64 system, namely Icon Packager...  various icons throughout the system are blank.  I mean blank  by the default icon is used.  Vista uses an icon that looks like paper with the corner folded over.

The first thing I tried to do is changing the Vista default icons to some theme but the result ended up setting the icons that were blank to the themes default blank icon.  I noticed the iconcache.db was not where it was supposed to be, but I assumed this was a Icon packager deal having to use it.  So I decided to uninstall icon packager to no avail.  I went to the iconcache.db that was replaced after uninstalling icon packager and deleted it hoping to rebuild the icon cache.  During all these steps I did the necessary reboots.

Nothing has worked and I am hoping someone knows how to fix this issue.

If you go to esupport.stardock.com and click on the downloads link, there is a fix posted for this that may help.  If it doesn't, write to [email protected]
