I actually have to apply the cursor set by double clicking the Theme package 2 times. the first time sets it to the default, the second time applying, it actually applies the theme to CursorFX..
yes the free version Does allow you to change Cursors..
Go here for full Scope of the difference between free and Pro. https://www.stardock.com/products/cursorfx/downloads.asp

I also use CFX free and as Tailsgirl pointed out, you cannot modify the cursor or use effects. You should be able to change from one cursor to another as often as you like by way of the CFX control panel or by double-clicking on the desired cursor file.If you can't then you may have a bad install.A little more information will be most helpful.
Edit:Too slow again,lol

Maybe neovision isn't compatible? [happens with some packs, for me anyway] have you tried applying any of the ones that came with the free? test them, just to be sure it's not just the pack/theme you downloaded

i had exactly the same problem, trouble was that none of the downloded cursors were being put in the correct folder (where all the defult ones are) so that even if you clicked apply on anyware except the main screen the cursor would not change. However when the file was moved to
C:\users\public\documents\stardock\cursorfx\themes
then the cursor will show up on the normal menu and can then be selected and applied like the default cursors
