Help with Iconpackager

I am having some problems. I reformatted and reinstalled Vista 64 because I made a mess of things "beta" testing stuff and just decided to start clean. However when I installed icon packager a bunch of my icons disappeard. Not system wide icons but several application icons. I have removed and re-installed icon-packager a few times (now I am getting errors about my serial number which I guess is too many installations, but be assured it is only on one computer). I did finally get things to re-load and most of the icons are back but I wonder what could have happened?
