basically you can start by designing a 'layout' in paint, paint.net, gimp, PSP, PS, whatever
cut out the various parts
start putting them together
when done, save the theme

Example: You design the Images for a window blinds . Cut them up in to the pieces necessary for each section of the WB theme. Then you would use SKS to assemble the images in to a working WB theme. When done you would use WB to apply the theme to your windows.
( the whole process is actually very much more detailed than the example here. This is but a generalization of the major steps to building a WB, each having many smaller steps along the way)..
