The Latest Packager & Vista

I'm brand new to Vista on a new computer. This morning I installed the latest Icon Packager and applied a theme for the first time on this computer, I chose 'spil's stuff ii'.
My problem is that I can only see the new icons for 'folders' if I have the view on 'list'. If I have them on 'medium' - as I prefer - or any other view they revert back to the default after a few seconds of seeing the newer ones, which look fine.
My other problem is that I have no icons in the start menu to the right side - run, control panel, search, printers, help, computer, etc.
They were never there and I had searched for a way to show them. I hoped that Icon Packager would put some there but it didn't.
Any help would be very much appreciated. Thanks.
