Icon Packager Not Changing All Icons

I'm using the Faux V2.5 icon set. A vast majority of the XP icons have changed to this package. However, sometimes when I'm get the "browse window" (i.e the one that pops up when you choose a save-to location in IE), the Desktop icon and the folders in the browse part of the window aren't changed. The Desktop icon goes back an forth, but the folder icons are always the XP default. What's going on here?
