How can I configure it?

The Wincustomize Website said that this would be highly customizable, but for the life of me I cannot figure out how.

I launched it by double-clicking on the .dream file, and it works fine in its single default configuration.

I do not have Stardock Central installed. Does this provide a front-end for what I am looking for, or should I be able to find a way to customize without it?

You need to open Personalization, select Wallpapers, and then select DeskScapes from the pull-down menu.  You should see all your installed .dreams (if not, then drag then .dream into that window). 

At the bottom of the window you will see options to configure the .dream.
